Reporting Doses Administered

Locations must report doses administered to CAIR (CAIR or CAIR/Healthy Futures) within 72 hours—for both private and publicly supplied vaccines. Providers screen patients using vaccine eligibility guidelines, and any written orders must indicate whether vaccinators should pull from private or public stock. Read more.

Reporting Options

Providers may report data to CAIR electronically using their EHR and data exchange, My Turn or CAIR Manual Entry. See Join CAIR (PDF) for reporting options.

Providers should have a backup system when conducting offsite clinics or any time the reporting system is not accessible. Providers may use the Vaccine Usage Logs (PDF) to collect administration data for later entry into CAIR.

Key Reporting Elements

These key elements must be reported:

  • IIS ID/Org Code (associated with the location’s myCAvax account)
  • patient race & ethnicity
  • NDC (identifies vaccine product administered)
  • funding source (identifies vaccine stock as VFC or non-VFC)
  • vaccine eligibility (criterion that patient satisfied to receive public doses)

Reporting on Vaccine Orders

Locations report doses administered (as well as doses on hand) on each vaccine order. Doses administered should match quantities reported to the immunization registry. Accountability data is verified by CDPH, and vaccine orders will not be approved until all doses are accounted for and any inventory discrepancies resolved.
